IntelliSpace PACS Alternatives and Philips IntelliSpace PACS Replacement Options

Philips publishes no IntelliSpace PACS price or end-of-life date, so we read the FDA, CISA, KLAS and federal award records: 17 VA awards that name the product, KLAS large PACS scores of 60.5 to 65.1, and what a renewal quote should list before you stay, move to Vue or replace it.

Decision support for qualified clinicians. Radiological.ai does not provide a diagnosis and is not a substitute for professional judgment.

The short answer: IntelliSpace PACS is the radiology PACS Philips built from iSite, the web PACS it bought with Stentor in 2005 for about $280 million. Philips still supports it on premises and sells it as a managed cloud service, but its newest product energy goes to HealthSuite Imaging, which Philips calls the next generation of Vue PACS, not of IntelliSpace. Philips publishes no price. A site with a renewal on the desk has three real options: stay and move to the cloud managed service, migrate to Vue or HealthSuite Imaging, or replace the PACS with another vendor. If the real pressure is radiologist time, a fourth option is to keep the PACS and add AI to the read.

Search for IntelliSpace PACS and the results are Philips documentation, a Canadian provincial user guide, load balancer vendors, a reseller and two software directories whose "alternatives" are dental and photo apps. Nobody independent lays out the decision. This guide is for the hospital, health system or VA imaging IT lead running IntelliSpace PACS 4.4 who has a support renewal, a cloud proposal or an RFP to write. Every figure below comes from a record you can check: FDA, CISA, KLAS public pages and USAspending, all pulled on October 5, 2026.

Who makes IntelliSpace PACS and where did it come from?

Philips makes it. The product began as iSite at Stentor, a Brisbane, California company whose technology came out of the University of Pittsburgh Medical Center. Philips agreed to buy Stentor in 2005 for roughly $280 million, about $45 million of which went to UPMC. iSite carried on under its own name until RSNA 2011, when Philips introduced IntelliSpace PACS 4.4 as the next release of iSite. The 4.4 line is still the current one. Public Philips documents run from build 4.4.550 in 2016 to 4.4.553, and Philips filed instructions for IntelliSpace PACS Anywhere R1.4 as recently as September 2025.

That history matters for one practical reason. Philips now owns two separate PACS code bases, because in March 2019 it also bought Carestream's health IT business and with it Vue PACS. Every Philips roadmap conversation you have is really a conversation about which of those two lines you end up on.

Is IntelliSpace PACS being discontinued?

Philips has published no end-of-life date for IntelliSpace PACS 4.4, and we looked through its document library, its press releases and the CISA advisories. The only public end-of-life statement is for the older iSite 3.6 platform, which CISA's November 2018 advisory describes as being at end of life and end of service. What Philips has said publicly, in June 2024, is that it will keep supporting on-premise IntelliSpace Radiology customers and will offer them HealthSuite Imaging "as an extension" of their managed services.

Customers read that differently. On KLAS's public comments page, a manager wrote in October 2025 that "IntelliSpace PACS is at the end of its life", and a director wrote in February 2026 that Philips "has been very wishy-washy on their direction. They say they are going to the Vue platform". Those are opinions, not Philips statements. They are still the conversation you will have with your own radiologists, so ask Philips for the support end date for your exact build in writing before you sign anything longer than a year.

What is the difference between IntelliSpace PACS and Vue PACS?

They are two different products from two different companies that Philips now owns. IntelliSpace PACS descends from Stentor's iSite and its iSyntax image streaming. Vue PACS descends from Carestream, and it is the base for both of Philips' newer products: HealthSuite Imaging in the cloud on AWS, and Image Management 15. Moving from IntelliSpace to Vue or HealthSuite Imaging is therefore a migration, with priors, interfaces, hanging protocols and training, even though the logo on the contract does not change.

How does IntelliSpace PACS score with KLAS?

Poorly in large hospitals, middling in small ones. In the Best in KLAS large PACS segment (300,000 studies a year and up), IntelliSpace PACS scored 60.5 in 2023, 63.7 in 2024 and 65.1 in 2025, and it was not ranked in any of those years. The 2024 report card gave it an F for culture and an F for loyalty. Sectra won the segment every year, scoring 91.0 in 2025. In the small PACS segment IntelliSpace scored 75 to 77 on limited data. The 2026 tables are behind a login, so we do not quote a 2026 score.

One trap worth knowing: the KLAS product page shows 75.5 and 80.1 next to IntelliSpace PACS. Those are the market average and the software average, not the product's score, and a few secondary sites have repeated them as if they were.

Not every comment is negative. An April 2026 KLAS comment calls IntelliSpace "a very stable platform" with "very little downtime". Stability is a real reason to stay, and it is the reason the fourth option below exists.

How much does IntelliSpace PACS cost?

Philips publishes no price, so the only checkable numbers are federal. We read every USAspending prime award that matched iSite, Stentor, IntelliSpace or Philips PACS. Only 17 awards name the product, all at the VA, totaling $2,423,269 with a median of $20,250. The largest is $1,240,417 for an iSite PACS system for VISN 11 in October 2010, and the most recent is $737,013 for IntelliSpace PACS maintenance in April 2021.

That small number hides the real spend. Another 286 awards to Philips entities, worth $154.4 million with a median of $298,774, describe PACS services or PACS maintenance without naming the product. Most of the older ones are very likely iSite or IntelliSpace, because the recipient is the former Stentor entity, but after 2019 the same recipient also sells Vue, so we do not count them as IntelliSpace spend. The useful takeaway for a buyer is the shape: a large VA network pays six or seven figures a year to keep a Philips PACS running, and the maintenance line is where the money sits. Our PACS pricing and cloud PACS cost guide benchmarks those line items across vendors.

What should an IntelliSpace PACS renewal quote list?

  • The support end date for your exact build, in writing, and what happens to security patches after it.
  • The cloud price against the on-premise price over five years, including storage growth, egress and the managed service fee.
  • Migration scope if Philips proposes Vue or HealthSuite Imaging: priors volume, interface rebuilds, iSyntax conversion and who pays for the dual-running period.
  • Security remediation status. CISA advisory ICSMA-18-088-01 covered all versions of iSite and IntelliSpace PACS with a CVSS score up to 10.0, and ICSMA-18-312-01 flagged default credentials. Ask which fixes are applied on your build.
  • Exit terms: data export format, timeline and cost if you leave at the end of the term.

That security line also tends to land on someone's desk outside imaging. A hospital running a legacy PACS with published advisories has to show which compensating controls cover it, and keeping that evidence current is easier with software that tracks each compliance obligation against its controls than with a spreadsheet nobody owns.

IntelliSpace PACS alternatives hospitals shortlist

Hospitals leaving IntelliSpace most often look at Sectra, which has won the KLAS large PACS segment every year we checked, then Agfa, Fujifilm Synapse, Visage and GE HealthCare. Each has its own trade-offs and we have a page for each: Sectra PACS alternatives and Sectra One cost, Agfa PACS system alternatives, Fujifilm Synapse PACS alternatives, Visage PACS alternatives and GE Healthcare Centricity PACS alternatives. Sites on another legacy platform from the same era can compare notes on McKesson PACS and Change Healthcare Stratus Imaging PACS.

Can you add AI to IntelliSpace PACS without replacing it?

Yes. An AI assistant receives studies from the PACS over standard DICOM and sends results back to the worklist and the reporting system, so it does not care whether the archive underneath is IntelliSpace, Vue or anything else. That separates two decisions that usually get bundled. The PACS decision is a multi-year migration with an RFP. The reading-room decision is about how many studies each radiologist gets through today, and it does not have to wait.

Radiological.ai is built for that second decision. It flags suspected findings for the radiologist's review, prioritizes the worklist so time-critical studies surface first, and drafts the structured report into your template. The radiologist reviews, edits and signs every study. How the assistant connects to your PACS covers the DICOM and HL7 plumbing, and pricing is published per reading radiologist on the pricing page: $399 a month on the Practice plan, or $199 a month billed annually. If you are writing a PACS RFP anyway, the full category view is on PACS software.

Common questions about IntelliSpace PACS

What replaced iSite PACS?

IntelliSpace PACS replaced iSite. Philips introduced IntelliSpace PACS 4.4 at RSNA 2011 as the next release of iSite, and FDA cleared IntelliSpace PACS 4.X under K111804 on July 28, 2011. iSite 3.6 is formally at end of life and end of service according to CISA's November 2018 advisory.

Is HealthSuite Imaging the successor to IntelliSpace PACS?

Not according to Philips. Philips launched HealthSuite Imaging in November 2023 as the cloud-based next generation of Vue PACS, the Carestream line. It offers HealthSuite Imaging to IntelliSpace Radiology customers as an extension of their managed services, which in practice means a migration between code bases.

Does IntelliSpace PACS have open FDA recalls?

No. openFDA lists 13 recall records for iSite and IntelliSpace PACS, all Class 2, and none is open. The most recent, Z-2338-2020, covered 26 IntelliSpace PACS 4.4 units where images could become corrupt. Open recalls under the IntelliSpace name belong to IntelliSpace Cardiovascular, a different product.

Is Philips selling its PACS business?

We found no sign of it. At its February 2026 Capital Markets Day Philips called itself number one in PACS and described Enterprise Informatics as a business it is scaling and moving to the cloud. Its 2025 divestment was Emergency Care, not imaging IT.
