Compare ยท Agfa PACS

Agfa PACS System Alternatives and What Agfa Enterprise Imaging Costs

The short answer

Agfa PACS is Agfa HealthCare's radiology PACS: the older IMPAX line and the newer Enterprise Imaging platform with the XERO viewer. It won Best in KLAS 2026 for small PACS. Federal buyers hold 680 Agfa PACS awards with a $97,668 median. The real choice is migrating IMPAX to Enterprise Imaging, moving to another PACS, or keeping the PACS and adding AI to the read.

Search for Agfa PACS and the results are Agfa HealthCare's own pages, product brochures and login screens. Nobody independent puts the numbers side by side, and the buyer usually shows up with an Enterprise Imaging Cloud proposal or an IMPAX support notice on the desk. This page is for a hospital, health system, VA or DoD imaging IT lead running IMPAX or Enterprise Imaging who has to decide whether to stay with Agfa, switch PACS, or keep the PACS and fix the read. Every figure comes from a public record we can point to.

Agfa HealthCare sells the imaging platform: IMPAX, Enterprise Imaging, the XERO universal viewer, the VNA and Enterprise Imaging Cloud. Radiological.ai sells none of that. It is an assistant that works alongside the PACS you already run, flags suspected findings as studies arrive, reorders the worklist and drafts the structured report for the radiologist to sign. You would replace Agfa with another PACS. You would add us to Agfa.

Agfa PACS against the PACS on a hospital shortlist

The choices an IMPAX or Enterprise Imaging site is usually weighing at renewal. Every row is a published fact or a public record, not a benchmark we ran.

What to compare Agfa IMPAX and Enterprise Imaging Other PACS (Sectra, Fujifilm Synapse, Visage, GE)
Who owns it Agfa-Gevaert (Belgium, listed). HealthCare IT is one of three divisions. IMPAX came largely from Mitra Imaging, bought January 2002. Sectra (Sweden), Fujifilm (Japan), Visage (Pro Medicus, Australia), GE HealthCare (US), which also owns Intelerad since March 2026.
Was it sold? No. Agfa sold its European hospital information business to Dedalus in May 2020 (EUR 975M enterprise value). US imaging IT stayed with Agfa, and we found no sale or spin-off since. GE closed its $2.3 billion Intelerad purchase on March 18, 2026. Check change-of-control terms with any vendor.
Product lines IMPAX (the older PACS), Enterprise Imaging for radiology and cardiology, XERO universal viewer, VNA. Each sells one main PACS line, except GE with Centricity, True PACS and Intelerad.
Cloud option Enterprise Imaging Cloud on AWS. Cloud was 51% of Agfa HealthCare IT orders in Q2 2026. Sectra One Cloud, Fujifilm Synapse on AWS, Visage 7 hosted, GE True PACS SaaS on AWS.
Newest FDA 510(k) K252199 Agfa HealthCare Enterprise Imaging, October 15, 2025. 19 image software (LLZ) clearances since 2001. Fujifilm K261713 Synapse PACS 7.6.0 (June 2026). GE K211312 Universal Viewer (2021).
FDA recalls 37 imaging software recall records from 2006 to 2024. Two XERO recalls still open, including Z-0055-2025 on 801 units (wrong localizer line on scouts). GE Centricity 55 recall events, 31 entries open. Check each vendor in the openFDA recall database.
Independent record Best in KLAS 2026: number one small PACS (93.2), XERO number one universal viewer for a third year, number one VNA, number two large PACS. Sectra has been Best in KLAS for US PACS for 13 years. Visage leads large academic sites.
Where it wins Deep federal experience (VA and the DoD DIN-PACS program), strong viewer and VNA scores, radiology and cardiology on one platform. Sectra on user satisfaction, Visage on speed for large health systems, GE on one vendor for scanners and PACS.
Switching cost Staying avoids a vendor change, but IMPAX to Enterprise Imaging is still a migration project. High. Priors migration, interfaces and training. Budget it as its own line.

Agfa facts come from Agfa-Gevaert financial releases, Agfa HealthCare press releases and the public KLAS award announcements, collected October 2, 2026. FDA data is from the openFDA 510(k) and recall databases. Federal award data is from USAspending.gov, award types A to D, fiscal 2008 to 2026, every recipient name and description read by hand.

What the public record shows

What federal buyers paid Agfa for PACS

Agfa publishes no PACS price, so we read every federal award to an Agfa recipient and kept the ones whose description names PACS, DIN-PACS, IMPAX or Enterprise Imaging.

Group of awards Count and value What it tells a buyer
Agfa PACS awards 680 awards, $306.36M Descriptions that name PACS, DIN-PACS, IMPAX or Enterprise Imaging. The largest imaging IT record of any vendor on this site.
Typical award Median $97,668 Quartiles $22,936 and $472,553. Most lines are annual maintenance, sustainment and support.
Largest award $16,572,400 (VA, May 2019) Agfa PACS for VISN 22, base year, running to October 2024. Next is $10,199,641 for VISN 20 commercial PACS services (2009 to 2015).
By agency VA 459 awards $237.40M DoD 175 awards $66.74M, almost all DIN-PACS through the Defense Logistics Agency. HHS 44 awards $2.18M, nearly all Indian Health Service.
Recent renewals $8,988,491 (VA, July 2024) Agfa PACS sustainment to July 2027. DIN-PACS maintenance orders of $4.42M (January 2025) and $4.92M (February 2026) show the DoD still renewing.

Source: USAspending.gov prime awards to Agfa HealthCare Corp, Agfa Corp and Agfa US Corp, award types A to D, fiscal 2008 to 2026, pulled October 2, 2026. 3,691 rows read by hand. Excluded from the PACS set: film, CR and DR hardware and consumables (1,781 awards), CD and DVD media writers, other imaging software such as dictation and dose monitoring (124 awards), and 1,106 rows whose description does not say what was bought.

Radiological.ai vs Agfa PACS, honestly

A fair look at what each does well. Both are capable tools. Here is where they differ.

What matters Radiological.ai Agfa PACS
What you are buying Software that flags suspected findings on the image, prioritizes the worklist and drafts the structured report. The PACS: IMPAX or Enterprise Imaging, with the XERO viewer, VNA and cloud hosting.
Relationship to your PACS Works alongside it. It reads from the PACS you already run, Agfa included. It is the PACS. A move to Enterprise Imaging or another vendor replaces what you have.
Unit of cost Per reading radiologist, published: $399 a month on the Practice plan or $199 a month billed annually, and $598 a month on the Group plan or $299 a month billed annually. Unpublished. Quote based. Federal Agfa PACS awards have a $97,668 median.
Project size Days to weeks. No image migration. Months. Priors migration, interfaces, archive and training.
Who signs the report The radiologist. The assistant never signs anything. The radiologist, in whichever reporting system you run with the PACS.
Best suited for Agfa sites whose radiologists need more studies read per hour before the PACS decision is made. Health systems that want radiology, cardiology, viewer and VNA from one imaging IT vendor.

Comparison reflects general, publicly understood positioning. Capabilities change, so check each product for the latest.

This page covers one PACS vendor. For the category, PACS software separates the deployment models, and PACS pricing and cloud PACS cost benchmarks quotes against federal PACS awards. If the RIS is up for renewal too, radiology information system software compares RIS vendors for hospitals and imaging centers.

If another hospital PACS is on your shortlist, Sectra PACS alternatives, Fujifilm Synapse PACS alternatives, Visage PACS alternatives and GE Centricity PACS alternatives work through those quotes the same way. If you also run Philips or Carestream, Vue PACS alternatives covers that renewal.

If the PACS is fine for now and the real ask is AI, how the assistant integrates with your PACS explains what connects to what, radiology worklist prioritization covers the triage side, and our pricing is published per radiologist.

Why groups pick Radiological.ai

One assistant across the whole read

Ask what happens to IMPAX

We found no public Agfa end-of-support date for IMPAX, but customers have described their IMPAX systems as end of life when moving to Enterprise Imaging. Ask for the support end date for your version in writing, and whether the move is priced as an upgrade credit or a new contract.

Read the open XERO recalls

Two XERO Viewer recalls are still open in openFDA, one covering 801 units on versions 8.0.0 to 8.4.1 where the localizer line can be placed wrongly on the scout. Ask which apply to your version and when the fix ships.

Price the cloud move against license

Agfa says its HealthCare IT revenue fell in 2026 as customers shift from perpetual licenses to cloud subscriptions. A subscription changes the five-year total. Compare it with what you already pay in maintenance, which federal awards put at a $97,668 median per line.

Separate the PACS decision from the AI decision

If your radiologists are short on time today, you do not have to wait for a PACS migration. An assistant that reads from IMPAX or Enterprise Imaging flags suspected findings and drafts the report now. Every flag is a prompt to look again and the radiologist signs every report.

Good questions

Agfa PACS vs Radiological.ai, answered

We found no public Agfa end-of-life notice for IMPAX. Agfa is moving customers to Enterprise Imaging and Enterprise Imaging Cloud, and some health systems have described their IMPAX systems as end of life during that move. Ask your account team for the support end date of your version in writing.
IMPAX is Agfa's older radiology PACS, built largely on software from Mitra Imaging, which Agfa bought in 2002. Enterprise Imaging is the newer platform that covers radiology, cardiology and other departments with the XERO viewer and a VNA. It received FDA 510(k) K252199 on October 15, 2025 and is sold on premises or as Enterprise Imaging Cloud on AWS.
Agfa does not publish PACS prices. Federal records give a real reference: 680 Agfa PACS awards totaling $306.36 million, with a median of $97,668 and quartiles of $22,936 and $472,553, mostly maintenance and sustainment. A new enterprise deployment adds licenses or subscription, archive, hosting, interfaces and migration on top.
Only part of it. In May 2020 Agfa sold its European hospital information business, including ORBIS, to Dedalus at an enterprise value of EUR 975 million. The imaging IT business in North America, including IMPAX, Enterprise Imaging and XERO, stayed with Agfa, and we found no sale or spin-off announced since.
Yes, in several segments. In Best in KLAS 2026 Agfa HealthCare ranked first for small PACS with a score of 93.2, first for universal viewer with XERO for the third year running, first for VNA and second for large PACS. KLAS scores reflect customer satisfaction, not price.
Hospitals replacing Agfa most often look at Sectra, Fujifilm Synapse, Visage, GE HealthCare and Philips. Imaging centers also look at Intelerad, RamSoft and Konica Minolta Exa. If the PACS itself works and the radiologists are the bottleneck, the alternative is to keep Agfa and add an AI assistant to the read.
Yes, mainly through RUBEE, the Agfa AI suite. RUBEE packages curated third-party algorithms and an orchestrator into the Enterprise Imaging workflow to flag and prioritize urgent cases and automate routine steps. The VNA stores the results and XERO displays them. Agfa notes that offerings vary by region, so ask which algorithms in your package are FDA-cleared for US use.
Yes. An assistant receives studies from IMPAX or Enterprise Imaging over standard DICOM and sends results back to the worklist and the reporting system. Radiological.ai flags suspected findings, reorders the worklist and drafts the structured report. It is decision support and the radiologist signs every report.
