iCAD ProFound AI Alternatives and Mammography AI Software Compared

The short answer

iCAD ProFound AI is mammography and tomosynthesis detection software, FDA-cleared since 2018. RadNet bought iCAD for about $103 million in July 2025 and folded it into DeepHealth, which sells its own Saige-Dx. The cleared alternatives are Hologic Genius AI, ScreenPoint Transpara, Therapixel MammoScreen, Lunit INSIGHT and DeepHealth itself.

Search for iCAD or ProFound AI and you get iCAD's own pages, investor coverage of the RadNet deal and a few press releases. Nobody independent lines up the clearances, the ownership change and the public contract record, and that is exactly what a breast center needs when the ProFound renewal arrives: an independent imaging center or hospital breast program that now buys its mammography AI from a subsidiary of a company that also runs imaging centers. Every figure below comes from a public record we can point to.

iCAD sells mammography and tomosynthesis detection, breast density and risk tools as point products for the breast center. Radiological.ai is an assistant across the whole read: it flags suspected findings on X-ray, CT, MRI and mammography, prioritizes the worklist and drafts the structured report, alongside whatever detection product you keep.

What to compare iCAD ProFound AI Other cleared mammography AI
Who owns it RadNet, through its DeepHealth AI subsidiary. RadNet paid about $103 million in stock (0.0677 RadNet shares per iCAD share) and closed in July 2025. iCAD brought nearly 70 staff and an installed base of more than 1,500 provider locations. Hologic (Genius AI), ScreenPoint Medical (Transpara), Therapixel (MammoScreen), Lunit (INSIGHT MMG and DBT, plus Volpara since 2024) and DeepHealth (Saige-Dx), which RadNet also owns.
First detection clearance K182373, December 6, 2018, PowerLook Tomo Detection V2, the product sold as ProFound AI for DBT. Transpara K181704 (November 2018), MammoScreen K192854 (2020), Genius AI K201019 (2020), Lunit INSIGHT MMG K211678 (2021), Saige-Dx K220105 (2022).
Most recent detection clearance ProFound Detection V4.0, K240417, November 8, 2024. Transpara K260898 (August 2026), MammoScreen 5 K260714 (June 2026), Saige-Dx K253825 (June 2026), Lunit INSIGHT DBT 1.2 K253796 (March 2026), Genius AI 2.0 K243341 (July 2025).
Breast density PowerLook Density Assessment, K180125 (2018) and V4.0 K211506 (2021). Volpara (now Lunit) K182310 and K211279, DeepHealth Saige-Density K243705 (December 2024).
Tied to a modality vendor No. ProFound runs on images from several mammography system makers. Genius AI is Hologic, which also makes the mammography units. The others are independent of the scanner.
Published price None. Federal orders show single-site support and license lines of roughly $9,000 to $28,000 a year. We found no published list price for any of the five.
Where it wins The longest tomosynthesis track record in the category, an installed base that radiologists already know, and density in the same family. Genius AI on Hologic fleets, Transpara and MammoScreen for independents who want a vendor that runs no imaging centers, Lunit where chest X-ray AI is also wanted.
Switching cost Staying avoids revalidation and retraining. Moderate. A new integration with the mammography units and PACS, a local validation period, and radiologists learning a new score.

FDA data is from the openFDA 510(k) database, product codes QDQ, LLZ and QIH, queried October 7, 2026. Deal terms come from the RadNet and iCAD merger announcement of April 15, 2025 and SEC filings. Federal data is from USAspending.gov.

What the public record shows

What federal breast centers pay iCAD

iCAD publishes no price. The only checkable US numbers are federal purchase orders, which are small, per-site and mostly support and license renewals.

Measure Figure What it tells a buyer
Prime awards to iCAD, 2008 to 2026 42 awards, $718,184 in total Federal mammography programs buy iCAD site by site, not through one enterprise contract.
Typical award Median $16,206, largest $45,425 Budget a renewal at one breast center in the tens of thousands, not hundreds of thousands.
Who buys Defense 19 awards ($328,734), VA 10 ($191,667), HHS 12 ($163,833), Justice 1 ($33,950) Military treatment facilities, VA medical centers and Indian Health Service hospitals such as Northern Navajo Medical Center.
ProFound by name VA, June 2022, $25,035 The one award that names ProFound in its description.
After the RadNet deal Defense $28,000 subscription license (June 2026), VA $9,359 annual support for 3D and 2D licenses (April 2026), HHS $15,497 3D mammography service (June 2026) Contracts still issue to ICAD, INC., so renewals continue under the old name. Side by side

Radiological.ai vs iCAD, honestly

A fair look at what each does well. Both are capable tools. Here is where they differ.

What matters Radiological.ai iCAD
What you are buying Software that flags suspected findings across the read, prioritizes the worklist and drafts the structured report. Detection and density software for mammography and tomosynthesis.
Studies covered X-ray, CT, MRI and mammography, under one subscription. Breast imaging only.
Unit of cost Per reading radiologist, published: $399 a month on the Practice plan or $199 a month billed annually, and $598 a month on the Group plan or $299 a month billed annually. Unpublished. License or subscription plus annual support, quoted per site.
Owner An independent software company that runs no imaging centers. RadNet, which also operates outpatient imaging centers.
Who signs the report The radiologist. The assistant never signs anything. The radiologist, in the reporting system beside the PACS.
Best suited for Practices that read breast and general studies and want one assistant across the whole worklist. Dedicated breast centers that want a long-established tomosynthesis detection product.

Comparison reflects general, publicly understood positioning. Capabilities change, so check each product for the latest.

This page covers one mammography AI vendor. RadNet also owns DeepHealth, which we cover on DeepHealth alternatives, and what RadNet ownership means for independent imaging centers explains the competitor-as-vendor question in more depth. For Lunit, which now owns Volpara, see Lunit alternatives. The two independent detection vendors most breast centers compare next are covered on Transpara alternatives and Therapixel MammoScreen alternatives.

For what the breast center buys around detection, mammography tracking software covers MQSA audit and follow-up, and MRS mammography reporting system covers the reporting side. AI mammography software shows how the assistant flags and drafts on breast studies.

If the question is cost across vendors, how much radiology AI costs compares per-study and subscription pricing, and the pricing page lists the per-radiologist plans.

Why groups pick Radiological.ai

One assistant across the whole read

Ask who owns your data now

Your ProFound contract was signed with iCAD and is now served by a RadNet subsidiary. Ask in writing whether your images or outcomes feed DeepHealth model training, and what the data use terms say after the change of control.

Ask about the product roadmap

RadNet now owns two cleared mammography detection products, ProFound and Saige-Dx. Ask which one gets new clearances, how long ProFound support is committed, and whether a migration to Saige-Dx would be priced as an upgrade or a new sale.

Benchmark against the federal record

Federal breast centers pay iCAD a median of about $16,000 per order, mostly annual support. If your renewal quote is far above that for one site, ask for the line items.

Separate detection from capacity

Detection marks a region and stops. If the real problem is how many studies each radiologist gets through, an assistant that also orders the worklist and drafts the report addresses that, and it can sit beside ProFound.

Good questions

iCAD vs Radiological.ai, answered

RadNet bought iCAD. The all-stock deal, worth about $103 million or $3.61 per iCAD share, was announced on April 15, 2025 and closed in July 2025. iCAD was folded into DeepHealth, the RadNet AI subsidiary that also sells the Saige-Dx mammography product.
Yes, as part of RadNet. ProFound AI is still sold and supported, and federal agencies were still issuing support and license orders to ICAD, INC. in 2026. iCAD no longer trades as an independent public company, so contract, roadmap and data questions now go to a RadNet subsidiary.
ProFound AI is FDA cleared, not approved, through the 510(k) pathway. The tomosynthesis detection product was cleared as K182373 in December 2018, followed by K191994 (2019), K203822 (2021) and ProFound Detection V4.0, K240417, in November 2024. Clearance means substantial equivalence, not proof of better outcomes.
iCAD publishes no price. Federal orders give a sense of scale: 42 awards to iCAD since 2008 have a median of $16,206, and recent single-site lines run from $9,359 for annual support on 3D and 2D licenses to $28,000 for a subscription license. Commercial quotes depend on units, volume and term.
The other FDA-cleared mammography detection products are Hologic Genius AI Detection, ScreenPoint Transpara, Therapixel MammoScreen, Lunit INSIGHT MMG and DBT, and DeepHealth Saige-Dx. Independent centers that do not want to buy from a RadNet company usually shortlist Transpara, MammoScreen and Lunit.
Yes. Saige-Dx came from DeepHealth, cleared five times between 2022 and June 2026, and ProFound came with iCAD in 2025. Ask your account team which product will receive new clearances and how long ProFound support is committed before you sign a multi-year renewal.
Yes. Detection and reading assistance sit side by side. Radiological.ai receives studies over standard DICOM, flags suspected findings for review, prioritizes the worklist and drafts the structured report. The radiologist reviews, edits and signs every study, and ProFound keeps running.
