Synapse PACS Alternatives to Fujifilm Synapse and What the Fuji PACS System Costs
Fujifilm publishes no price for Synapse PACS, so we read all 202 US federal awards that name it: $78.2 million, a $64,615 median, and what a Synapse quote should itemize. Plus Synapse against Sectra and RamSoft on FDA clearances and pricing, and when adding AI beats migrating the PACS.

The short answer: Synapse PACS is the enterprise imaging platform sold in the US by FUJIFILM Healthcare Americas, and Fujifilm publishes no price for it. The best public benchmark is the federal record: 202 US government prime awards that name Synapse, PACS or the Fujifilm VNA, worth $78.2 million, with a median award of $64,615. The alternatives usually shortlisted against it are Sectra, Intelerad, Merge and, for outpatient centers, RamSoft. If the reason you are shopping is AI help with the read, you may not need to leave Synapse at all.
Product facts come from Fujifilm's US and EMEA product pages and press releases. FDA data is from the openFDA 510(k) database. Federal award figures are from USAspending.gov, award types A to D, fiscal 2008 to 2026, pulled September 27, 2026, with every recipient name and description read by hand.
Who makes Synapse PACS?
Fujifilm. The US business is FUJIFILM Healthcare Americas Corporation, based in Lexington, Massachusetts. That name dates from July 1, 2021, when Hitachi Healthcare Americas was renamed after Fujifilm completed its 179 billion yen purchase of Hitachi's diagnostic imaging business on March 31, 2021. The deal added Hitachi's CT, MRI, X-ray and ultrasound lines to Fujifilm, which already sold Synapse and, through its Sonosite unit, point-of-care ultrasound.
That matters to a buyer for one reason. A Synapse quote now comes from a company that also sells you scanners, so imaging IT and modality purchases can be bundled into one negotiation. Sometimes that gets you a better price. Sometimes it hides the software price inside a hardware deal where you cannot benchmark it.
Which Synapse product are you being quoted?
Fujifilm now uses "Synapse Enterprise Imaging" as an umbrella brand, and the product names overlap. Settle which one is on the proposal before comparing anything.
| Product | What it is | Who it is aimed at |
|---|---|---|
| Synapse PACS | Web-based radiology PACS with embedded 3D and AI, also covering mammography, cardiology, pathology and point-of-care ultrasound | Hospitals and health systems |
| Synapse 7x | One zero-footprint viewer across radiology PACS, cardiology PACS, 3D, VNA and the REiLI AI platform | Enterprises running several Synapse modules |
| Synapse VNA | Vendor neutral archive for DICOM and non-DICOM content from any department | Multi-site systems consolidating archives |
| Synapse Mobility | Zero-footprint universal viewer with cloud image sharing | Referring physicians and clinicians outside radiology |
| Synapse One | All-in-one product launched at RSNA 2025: patient portal, self-scheduling, RIS, revenue cycle options, PACS, 3D and physician portal, running on AWS | Outpatient imaging centers |
| Synapse on AWS | Radiology, cardiology and pathology PACS plus the VNA, validated for AWS under a March 2025 agreement, with a SaaS listing on AWS Marketplace | Buyers who want the platform hosted rather than on premises |
Fujifilm also sells a second, smaller cardiology product that is easy to confuse with the PACS lines above. Synapse Select is the cloud cardiovascular information system Fujifilm inherited as VIDIStar when it bought Hitachi's imaging business in 2021, aimed at community hospitals, practices and imaging centers. If that is the contract on your desk, VIDIStar PACS alternatives covers its FDA record and the CVIS rivals.
How much does Synapse PACS cost?
Fujifilm does not publish a rate card. Pricing is by quote, and Fujifilm has historically priced PACS on procedure volume and planned concurrent users rather than per seat. Commercial quotes are private, so the only checkable prices are what the US government has paid, which is public on USAspending.gov.
We pulled every federal prime award to a Fujifilm medical entity since fiscal 2008, set aside the Sonosite ultrasound orders and the X-ray, film and MRI hardware, and kept the 202 awards whose description names PACS, Synapse, picture archiving, the VNA or TeraMedica, the archive company Fujifilm owns.
| Buyer | Awards | Total | Median award |
|---|---|---|---|
| Department of Defense | 114 | $72,944,525 | $182,325 |
| Department of Veterans Affairs | 37 | $2,596,385 | $43,336 |
| Health and Human Services | 26 | $1,666,851 | $40,405 |
| Department of Justice | 22 | $832,800 | $43,580 |
| All agencies (incl. NASA, 2) | 202 | $78,179,823 | $64,615 |
The middle half of those awards falls between $24,143 and $304,938. Twenty-three of them are over $1 million, and the five largest are all Defense Department orders under DIN-PACS, the military's digital imaging network program, topped by $5,561,535 in December 2023. Eighty of the 202 awards are DIN-PACS orders, which is why Defense holds 93 percent of the dollars.
Read those figures for what they are. A federal award is often one year of maintenance, one site's expansion or one server refresh, not a full platform purchase, so the $64,615 median is a typical transaction, not a typical total cost. Two patterns are still useful at the negotiating table. First, the VA, HHS and Justice medians all sit between $40,000 and $44,000, which is what routine Synapse support and add-on work costs a single facility. Second, the record keeps going: awards started in every year from 2007 to 2026, and 2025 brought 15 separate Defense orders for back-up servers alone. Synapse is a long relationship with steady line items, and you should price it that way.
For the category-wide picture, our PACS pricing and cloud PACS cost guide benchmarks quotes against federal PACS awards from every vendor, not just Fujifilm.
What should a Synapse quote itemize?
- The license basis. Annual procedure volume, concurrent users or both, and the price per study above and below your committed volume.
- Each module separately. Radiology PACS, cardiology, 3D, VNA, Mobility and any REiLI AI modules, each with its own line, so you can drop one at renewal.
- Hosting. On premises, Fujifilm hosted or AWS. If it runs on AWS, who pays the storage and egress bill as your archive grows.
- Hardware. Servers and workstations, with the refresh schedule. The federal record shows back-up servers bought as separate orders years after go-live.
- Migration and interfaces. Priors migration, HL7 interfaces to the RIS and EHR, and the reporting integration, each priced.
- Maintenance and uplift. The annual support percentage and a cap on the renewal increase.
- Exit terms. How images, priors and reports are exported at the end, in what format, how fast and at whose cost.
The hosting line deserves more attention than it usually gets. A cloud PACS moves archive growth from a capital budget into a monthly bill, and imaging archives only grow. If Synapse is going onto AWS, put it in the same place you watch the rest of your infrastructure bill, whether that is your finance team's spreadsheet or a tool for tracking cloud and SaaS spend across vendors, so the storage line gets looked at every month rather than at renewal.
Synapse PACS vs Sectra vs RamSoft
| Fujifilm Synapse | Sectra PACS | RamSoft | |
|---|---|---|---|
| Typical buyer | Hospitals, health systems, military and VA imaging | Hospitals and health systems | Independent imaging centers, teleradiology groups |
| How it is sold | Quote, volume and concurrency based, on premises or AWS | Sectra One subscription, including Sectra One Cloud on Azure | PowerServer (cloud or on premises) and cloud-native OmegaAI on Azure |
| Price published | No | No | No |
| Public price benchmark | 202 federal awards, $64,615 median | One disclosed contract: $21M over five years (August 2025) | None found |
| Latest PACS FDA 510(k) | K261713, Synapse PACS 7.6.0, June 18, 2026 | K081469, IDS7 v11.3, June 12, 2008 | K222476, OmegaAI Image Viewer, November 2022 |
| Own AI | REiLI platform, plus Synapse Lung Nodule AI (K254075, May 2026) | Amplifier marketplace, Oxipit chest X-ray AI acquired April 2026 | AI reporting with voice dictation in OmegaAI |
Fujifilm is the most active of the three at the FDA by a wide margin. Its Synapse filings run from K051553 in 2005 to three clearances in 2026 alone: Synapse 3D Base Tools V7.2 in April, Synapse Lung Nodule AI in May and Synapse PACS 7.6.0 in June. Sectra's PACS workstation line was last cleared in 2008, though Sectra is also the vendor with the strongest satisfaction record, 13 consecutive years of Best in KLAS in the US. Our Sectra PACS cost and alternatives page covers that side of the shortlist, and RamSoft PACS alternatives covers the outpatient option.
Should you replace Synapse, or add AI to it?
Replace it when the platform itself is the problem: the version is out of support, the archive cannot scale, uptime is poor or the vendor relationship has broken down. That is a project measured in months, with priors migration, new interfaces and retraining every radiologist, and it should be run as one.
Do not replace it because the radiologists want AI. That is the most common reason a PACS review starts and the worst reason to finish one with a migration. Fujifilm sells its own REiLI AI modules, and independent assistants read studies from Synapse over standard DICOM and send results back, so the radiologist keeps reading in the viewer they already know.
That second path is where we fit. Radiological.ai works alongside the PACS you already run, Synapse included: it flags suspected findings as studies arrive, moves them up the worklist and drafts the structured report for the radiologist to review and sign. It is decision support and the radiologist always signs. How the assistant integrates with your PACS explains the connection, and our pricing is published per radiologist seat, which is more than any PACS vendor on this page will tell you.
Is Synapse PACS cloud based?
It can be. Synapse PACS is web based and has been sold on premises for most of its life, but since a March 2025 strategic agreement with AWS, Fujifilm offers the radiology, cardiology and pathology PACS and the Synapse VNA hosted on AWS, and Synapse One for outpatient centers runs on AWS by design. Ask which deployment the quote assumes.
Is Fujifilm Synapse FDA cleared?
Yes. Synapse PACS holds FDA 510(k) clearances including K160108 (2016), K190232 (2019), K243647 for version 7.5 (June 2025) and K261713 for version 7.6.0 (June 2026), plus a long run of Synapse 3D clearances and K254075 for Synapse Lung Nodule AI (May 2026). Check that the version you are quoted matches a cleared one.
What are the alternatives to Fujifilm Synapse PACS?
For hospitals and health systems: Sectra, Intelerad, Merge PACS (now part of Merative), Visage 7 and the Philips and GE Healthcare platforms. For outpatient imaging centers: RamSoft, Novarad and cloud-first vendors. For a full vendor list with switching reasons, see the radiology software alternatives hub, and for the category itself, PACS software compares the deployment models.
