Merge PACS Alternatives for Hospitals on Merative PACS, Cost, Recalls and What to Switch To

Merge publishes no price for Merge PACS, so we read all 136 US federal awards to Merge entities: $10.9 million, a $16,988 median, and 58 percent of it for cardiology. Plus who owns Merge now, the two open 2026 recalls, what a renewal quote should itemize, and Merge against Sectra, Synapse and Visage.

The short answer: Merge PACS is the radiology PACS from Merge Healthcare, a business of Merative, the company Francisco Partners formed from IBM Watson Health in 2022. Merge publishes no price. The US federal record shows 136 prime awards to Merge entities worth $10.9 million, with a median award of $16,988, and most of that money went to cardiology products rather than radiology PACS. The alternatives most often shortlisted are Sectra, Fujifilm Synapse, Visage 7 and Intelerad, or keeping Merge and adding AI to the read.

Ownership facts come from SEC filings, PR Newswire and Merative press releases. FDA data is from the openFDA 510(k) and recall databases. Federal award figures are from USAspending.gov, award types A to D, fiscal 2008 to 2026, pulled September 28, 2026, with every recipient name and description read by hand.

Who owns Merge PACS now?

Merge has changed hands twice since 2015, and each change shows up in how the product is sold and supported. Merge Healthcare, based in Hartland, Wisconsin, bought AMICAS in April 2010 for about $224 million at $6.05 a share. IBM then bought Merge, closing on October 13, 2015 at $7.13 a share in cash, about $1 billion. At the time Merge was in use at more than 7,500 US sites, and it became the imaging arm of Watson Health.

In January 2022 IBM agreed to sell the Watson Health data and analytics assets to Francisco Partners, a private equity firm. The deal closed on June 30, 2022, reportedly for about $1 billion, and the new company was named Merative, headquartered in Ann Arbor. Merge sits inside it alongside Micromedex, Truven and Cúram. Since then the imaging business has been rebranded as a standalone "Merge" with its own website, and on September 15, 2026 it launched MergeIQ, an AI enterprise imaging platform. We found no announced sale or spin-off of Merge through September 2026.

Why a buyer should care: a private equity owner running a portfolio of health data businesses has different priorities from IBM or from a pure imaging company. Ask where Merge sits in Merative's plans, what the product roadmap is for the version you run, and what happens to your contract if the imaging unit is sold.

Which Merge product are you running?

The product names have shifted with every owner, and older names still appear in FDA records and support contracts. Settle which product is on your contract before comparing anything.

How much does Merge PACS cost?

Merge does not publish a rate card, and no independent source we could find publishes a real Merge PACS price. The only checkable prices are what the US government has paid, which is public on USAspending.gov. We pulled every federal prime award to Merge Healthcare Solutions, Merge Healthcare and AMICAS since fiscal 2008, then removed rows that only matched the name. That removed 23 Merative rows, all for claims data and drug reference products rather than imaging, plus a technology firm and an air museum that happened to contain "merge" in their names.

The median award is $16,988, and the middle half of awards falls between $4,411 and $60,372. Not one award reaches $1 million. The largest is $861,479 in 2020 for Merge Vericis cardiology maintenance at the Oklahoma City VA, followed by a $625,179 Merge system upgrade in 2023 and a $603,219 Merge Hemo support order in 2016. Awards that mention cardiology, hemodynamics, Vericis, the cath lab or the heart account for $6.32 million, 58 percent of the total. Radiology PACS and eFilm orders are mostly small maintenance and license lines.

Two things follow for a negotiation. First, federal Merge spending is maintenance-heavy, which fits a mature installed base rather than new platform sales. Second, if your Merge footprint includes cardiology, that is probably where most of the money goes, so price radiology and cardiology as separate lines and do not let a renewal bundle hide which one is growing. For the category-wide picture, our PACS pricing and cloud PACS cost guide benchmarks quotes against federal PACS awards from every vendor.

What should a Merge renewal or replacement quote itemize?

  • The license basis. Study volume, concurrent users or named sites, and the price above and below your committed volume.
  • Radiology and cardiology separately. Merge PACS, Merge Cardio, Merge Hemo and the VNA each on their own line, so you can drop or replace one at renewal.
  • The upgrade path. Which version you land on, whether the move from Unity or an older Merge PACS release is priced as an upgrade or as a new sale, and who pays for the data migration.
  • Hosting. On premises or Merge cloud, and if cloud, who pays storage growth as the archive expands.
  • Support and uplift. The annual maintenance percentage, a cap on the renewal increase and the end-of-support date for your version, in writing.
  • Change of control. What happens to price, support and your exit rights if Merge is sold.
  • Exit terms. How images, priors and reports are exported, in what format, how fast and at whose cost.

The dates in that list are easy to lose across a multi-year contract, especially renewal notice windows and support end dates. Keep them where you track the rest of your vendor obligations, in a proper contract management system with renewal alerts, so the notice period does not lapse while the team is still comparing vendors.

Are there open recalls on Merge PACS?

Yes, two as of September 2026, both Class 2 and both open in the FDA recall database. Z-2874-2026, posted June 30, 2026, covers Merge PACS and OrthoPACS 9.4 through 9.4 P02: thumbnails from a previously opened patient's study can appear while a different patient's study is open, across 53 units in the US, Canada and the UK. The published workaround is to turn off automatic Thumbnail Bar refresh. Z-3004-2026, posted July 22, 2026, covers Merge PACS 9.5, where ultrasound measurements may display inaccurately, across 20 units.

Recalls are routine for any software medical device, and openFDA lists 96 recall records under Merge Healthcare from 2011 to 2026 across all its products. The practical question is whether your version is affected, whether the fix is installed, and whether the workaround changes how radiologists read. Ask for that in writing before the renewal.

Should you replace Merge PACS, or add AI to it?

Many Merge sites start shopping because the radiologists want AI help in the read, then find themselves quoting a full PACS migration. Those are two different projects with very different price tags. A PACS replacement means priors migration, new interfaces to the RIS and EHR, rebuilt hanging protocols and retraining, over months. Adding AI means connecting an assistant that reads from the PACS you already run, over standard DICOM, in days to weeks.

Replace Merge if the platform itself is the problem: an unsupported version, a viewer that is slow on large studies, or a roadmap you no longer trust. Keep it and add AI if the PACS works and the real complaint is reading volume. Radiological.ai does the second job. It flags suspected findings as studies arrive, moves the urgent ones up the worklist and drafts the structured report, and the radiologist reviews, edits and signs every study. It is priced per reading radiologist and the radiology AI pricing is published. How the assistant connects to your PACS shows what connects to what.

Is Merge PACS FDA cleared?

Yes. Merge PACS holds 510(k) clearances K173475 from December 2017 and K192455 from October 2019, and the Merge Universal Viewer was cleared as K250301 in April 2025. Earlier clearances sit under the names Merge Technologies, Cedara and AMICAS, going back to 1990. Ask which clearance covers the exact version on your quote.

Is Merge PACS cloud based?

It can be. Merge sells both on-premises and cloud deployments. In June 2025 MultiCare Health System, with 13 hospitals, announced an expansion to Merge cloud PACS. Many long-standing Merge sites still run on premises, so a move to the cloud is often priced as part of an upgrade rather than a simple renewal.

What are the alternatives to Merge PACS?

For hospitals and health systems: Sectra, Fujifilm Synapse, Visage 7 and Intelerad, with the Philips and GE Healthcare platforms also in some evaluations. For outpatient imaging centers: RamSoft, Novarad and cloud-first vendors. For a full vendor list with switching reasons, see the radiology software alternatives hub, and for the category as a whole, PACS software separates the deployment models.
