What is PowerScribe 360?

PowerScribe 360 is a radiology reporting platform: the software a radiologist uses to create the diagnostic report after reading a study. Its core job is turning dictated speech into a finished, structured report. A radiologist opens a case, reviews the images in the PACS viewer, speaks the findings and impression into a microphone, and PowerScribe converts that speech into text laid out in the group's report template. The radiologist edits and signs, and the signed report becomes part of the patient record and flows back to the ordering physician.

It came from Nuance, the speech-recognition company Microsoft acquired and folded into its healthcare division. "360" is the on-premises generation of the product line, installed on servers the imaging group owns and licensed as a purchase with annual maintenance. It has been one of the most widely deployed reporting systems in US radiology, which is exactly why its retirement is such a large event for the field.

It is worth being precise about what PowerScribe 360 is not, because the confusion costs groups real evaluation time. It is not a PACS and it does not display images; you read in your viewer and report in PowerScribe. It is not the radiology information system either, which is the system that schedules the patient and produces the worklist the study arrived on. It is not an image-analysis product, so it does not look at pixels or flag findings. And it is not a worklist engine in its own right, though it integrates with one. Its territory begins the moment you start speaking and ends when you sign, which is a narrower slice of the shift than the brand's prominence suggests.

The people who actually touch it are radiologists dictating, transcriptionists or editors in groups that use delegated editing, and an administrator who maintains the templates, AutoText library and interfaces. That last role is the one groups underestimate, and it is usually the person who discovers what a migration really costs.

How does PowerScribe 360 work?

The workflow is built around voice. When a radiologist speaks into the dictation microphone, PowerScribe uses the Dragon Medical speech recognition engine to transcribe the words in real time and drop them into the correct sections of a report template. Groups can run it a few ways: straight speech recognition with self-editing, speech recognition with delegated editing where a transcriptionist cleans up the draft, or macro-driven and auto-structured reporting where natural language understanding organizes the dictated content into a standard structure automatically.

Templates and AutoText are where the real value accumulates. Over years, a group builds a library of report templates and voice-triggered text snippets that encode how it likes to report each study type. That library is usually the single thing a group is most afraid of losing in any migration, because it represents thousands of hours of standardization. The assist, though, begins only once the radiologist is already reading and already talking. Deciding which study to read next, and catching the subtle finding, remains entirely the radiologist's job. Who makes PowerScribe 360?

Microsoft. The product was created by Nuance Communications, the company behind Dragon speech recognition. Microsoft acquired Nuance in 2022 and now sells and supports the PowerScribe line through its healthcare group. That matters for two reasons: the roadmap is set by Microsoft's priorities, which currently point firmly at cloud and subscription products, and the same company also owns Dragon and the DAX ambient documentation product, which are related but not the same thing.

What is the difference between PowerScribe and Dragon?

Dragon is the speech recognition engine; PowerScribe is the radiology reporting application built on top of it. Dragon Medical listens and turns speech into text in any clinical setting. PowerScribe wraps that transcription in everything radiology reporting specifically needs: report templates, which PowerScribe calls AutoText, structured sections, worklist integration and the sign-off workflow. Put simply, Dragon is the ears and PowerScribe is the radiology-specific product around them. Microsoft's DAX is a third, different thing again: it listens to a spoken patient encounter and drafts a clinical note, so it has nothing to transcribe in a diagnostic image read. Is PowerScribe 360 being discontinued?

Yes, in the way that matters to you. Microsoft has told customers that annual renewals and maintenance for PowerScribe 360 ended on August 31, 2026, and that full support ends on August 31, 2027. End-of-life notices went out in early 2026. The binaries will keep running after those dates, but there is no path to renew maintenance or get supported updates, and running unmaintained clinical software that handles protected health information becomes a compliance exposure your security team has to track and document rather than a decision you can quietly defer. For most groups, "we will just keep running it" stops being a real option.

What is the difference between PowerScribe 360 and PowerScribe One?

Three things change. PowerScribe 360 runs on your own servers and was bought as a license with annual maintenance; PowerScribe One runs in Microsoft's cloud on a recurring subscription. The speech engine moves to the cloud too, which vendor materials describe as removing the per-radiologist voice-profile training 360 sites are used to. And uptime becomes the vendor's responsibility plus your internet connection, rather than your local network.

What replaced PowerScribe 360, and what are the options?

Microsoft's answer is PowerScribe One, and for many groups that will be the path of least resistance: a familiar product family, a supported migration route that reuses existing 360 templates and content, and a vendor relationship already on paper. There is nothing wrong with choosing it. But a forced migration is the one moment in a decade when the cost of leaving is roughly the same as the cost of staying, because you do the template, integration and training work either way.

That symmetry is worth using. Three paths are genuinely open: move to PowerScribe One, move to another speech-based reporting platform, or ask whether dictation should still be the core of the reporting layer at all. Is PowerScribe 360 cloud-based?

No. PowerScribe 360 is on-premises software, installed and maintained on servers your organization owns and runs inside your own network. That is precisely what is changing: PowerScribe One moves reporting to the cloud. The on-premises model has one advantage worth naming, which is that an internet outage does not stop dictation because everything runs locally. The cloud model trades that for automatic updates, vendor-managed infrastructure and no local servers to maintain. Which matters more depends on your connectivity, your IT staffing and your tolerance for depending on a vendor's availability.

How much does PowerScribe 360 cost?

There is no published price. Microsoft quotes PowerScribe per site through a reseller, sized against reader count, study volume and integration scope, which is why the estimates circulating online disagree with each other by an order of magnitude. What microphone or headset does PowerScribe 360 use?

Handheld dictation microphones rather than headsets, and the buttons are the reason: record, stop and field navigation all sit under your thumb, which a headset cannot do. Microsoft supports a small named list of PowerMic and Philips SpeechMike devices. The practical consequence for a migration budget is that existing microphones carry over to PowerScribe One, so a hardware refresh is not a line item.

What does PowerScribe cost, and what does Dragon cost?

Neither has a published price, and the two get quoted together often enough that it is worth separating them. The only verifiable figures for either are US federal contract awards, which are public record. PowerScribe pricing works through 183 of them for the reporting product.
