Microsoft acquired it in 2022 and has been folding the healthcare portfolio into its own branding since, so "a Nuance alternative" means different things depending on which product you run. Radiology groups are usually replacing one of three: PowerScribe 360, the on-premises reporting platform whose renewals and maintenance ended August 31, 2026; PowerScribe One, its cloud subscription successor; or the ambient documentation product formerly sold as DAX, which Microsoft merged with Dragon Medical One under the Dragon Copilot brand in March 2025. The credible alternatives differ for each. Solventum Fluency for Imaging is the other enterprise reporting platform, Rad AI and Sirona Medical come at reporting from the AI side, and Radiological.ai flags suspected findings, prioritizes the worklist and drafts the structured report on top of the PACS you already run. It is decision support, and the radiologist reviews, edits and signs every study.

The first thing worth saying is that you are probably not shopping for a Nuance alternative at all. You are shopping for a Microsoft alternative. Microsoft closed its acquisition of Nuance in 2022, and the branding has been dissolving ever since: DAX and Dragon Medical One were pulled together under the Dragon Copilot name in March 2025, and Microsoft now describes a radiologist experience inside Dragon Copilot that works alongside PowerScribe One. The Nuance name still appears on contracts and support portals, which is why the search term persists, but the product line behind it has moved.

That matters because the alternatives are completely different depending on what sits on your desk. A group running PowerScribe 360 has a deadline problem: annual renewals and maintenance ended August 31, 2026 and full support ends August 31, 2027, so the decision is being made for them. A group already on PowerScribe One has a cost and scope question instead, not an urgent one. A practice looking at ambient documentation is in a third market entirely, and in diagnostic radiology it is usually the wrong market, because there is no patient conversation in the reading room for an ambient tool to capture.

No. PowerScribe One is the vendor path and it is a legitimate choice, particularly if your organization is standardizing on Microsoft clinical tooling. Because the migration work happens either way, it is worth pricing at least one alternative before you sign, if only to know what your renewal negotiation is worth.

Microsoft does not publish list pricing for PowerScribe One or Dragon Copilot, so any figure quoted publicly is somebody else deal rather than a rate card. Pricing is scoped per site by reader count, volume and integration work. Ask for the fully loaded first-year total including interfaces, template migration and training, and then what the subscription renews at in year three once it is embedded.

Rarely for the diagnostic read, because ambient tools work by listening to a conversation between a clinician and a patient, and there is no such conversation when a radiologist reads a study alone.
